Blue Shield of California Q1 2027 Small Business update
For payroll and HR administrators preparing January renewals, employee communications, and benefit deduction changes after plan selection.
Published . Announcement reviewed on this date; this is not a claimed carrier publication date.
Official source: Blue Shield of California Q1 2027 Small Business product-cycle update →Blue Shield defines California Small Business as 1–100 employees. Its announced Q1 2027 statewide average base rate action is 3.4%. This base action is distinct from the posted average renewal changes in the October 19 rate-table update below. It is not your group’s renewal quote and not a payroll budget. Use the actual group renewal and completed plan elections before setting employer contributions or employee deductions.
Original carrier announcement facts come from the official product-cycle update linked above; the added renewal averages come only from Blue Shield’s official rate-table page linked below. This is a general administrative readiness checklist, not a plan recommendation or client-specific advice; exact coverage and cost-sharing depend on the applicable plan documents.
October 19, 2026 rate-table update
Rate tables reviewed ; this is our update date, not a claimed carrier posting date. The figures below apply to Q1 2027, January 1–March 31, 2027.
Official source: Blue Shield Small Business rate tables →The announced 3.4% Q1 2027 base rate action is not the same measure as Blue Shield’s posted average Q1 2027 renewal change. Keep these figures separate; do not add them together or substitute one for your actual renewal.
| Measure | Average |
|---|---|
| Announced Q1 2027 medical base rate action | 3.4% |
| Average Q1 2027 HMO renewal change | 12.3% |
| Average Q1 2027 PPO renewal change | 14.3% |
| Average Q1 2027 renewal change — all medical plans | 13.9% |
| Average Q1 2027 dental renewal action | 7.5% |
| Average Q1 2027 vision renewal action | 0% |
These are portfolio averages, not a client’s quote or a payroll budget. Actual small-group renewal outcomes vary by plan, rating region, age/demographic changes, and billable dependent changes. Blue Shield notes that aging can affect premiums and that, for subscribers with more than three children, a child turning 21 can become an additional billable member.
These posted figures are not a statement of regulatory approval and do not describe claim-experience rating for an individual group. Update medical, dental and vision payroll deductions only after reviewing the actual renewal and completed employee elections, using confirmed premiums and the employer’s contribution policy. A 0% average vision action does not guarantee an unchanged individual premium or deduction.
January renewal calendar
Announced schedule for January 2027 renewals. Blue Shield says these dates are subject to change.
- Renewal kits scheduled in Shield Renewals
- Group-level changes scheduled in the Employer Enrollment Tool
Review January renewals with your broker when released. Keep the renewal effective date, plan-selection decisions, enrollment readiness, employee communications, and payroll setup on the same calendar.
Announced 2027 plan changes
- Rx Spectrum pharmacy network — effective January 1, 2027
- The Rx Spectrum tiered pharmacy network expands across HMO and PPO off-exchange and Mirror plans. Verify pharmacies, preferred tiers, and prescription coverage for the selected plan before communicating employee options.
- HDHP telehealth — beginning in 2027
- Deductibles for telehealth services will be waived on high-deductible health plans (HDHPs). A waived deductible does not by itself mean every telehealth service is free; review the plan’s applicable member cost-sharing.
- Mental health and substance-use outpatient services — 2027 applicable plans
- Select mental health and substance-use outpatient services move to no cost share where applicable. Confirm the services and terms in the selected plan rather than assuming this applies universally.
- Plan names and member cost-sharing — 2027 plan designs
- Some Mirror HMO/PPO and HDHP/HSA-qualified PPO Savings plan names, deductibles, out-of-pocket limits, and other cost shares change. Compare the current and renewal plan documents; a familiar carrier or similar plan name does not establish unchanged benefits.
Virtual Blue standalone PPO withdrawals — at renewal in 2027
The listed Virtual Blue standalone PPO plans are withdrawn for 2027. Blue Shield says groups receive a 90-day notification before renewal and are mapped at renewal to the listed equivalent PPOs with Virtual Blue program enhancements. Groups can opt out of the recommended replacement and choose another available marketed plan.
| Withdrawn plan | Recommended replacement |
|---|---|
| Virtual Blue Platinum PPO 250/20 | Platinum PPO 250/15 |
| Virtual Blue Gold PPO 1500/45 | Gold PPO 1000/30 |
| Virtual Blue Silver PPO 2850/75 | Silver PPO 2550/75 |
| Virtual Blue Bronze PPO 7500/75 | Bronze PPO 7500/65 |
Compare the mapping, network, member cost-sharing, and available alternatives before plan selection. An equivalent replacement is not a guarantee of identical benefits or cost.
Medicare Part D creditable status — review at renewal
Blue Shield announces that some plans will no longer be Medicare Part D creditable upon renewal in 2027. Review the actual plan’s creditable status for Medicare-eligible employees and dependents; do not assume it carries forward from 2026.
The official update describes two options for affected groups: keep the plan and notify employees that its prescription coverage is not Medicare creditable, or select an alternative plan that is creditable. Coordinate plan review and employee notices with your benefits broker, including communications about potential Medicare Part D late-enrollment penalties.
Payroll and HR readiness
- Review the released renewal. Compare the group’s actual January renewal, any plan mapping, member cost-sharing, network, pharmacy tiers, formulary, and effective date with your broker.
- Prepare the employee census. Confirm enrollment and dependent information, coverage tiers, and pending employee changes so plan selection and enrollment files reflect the group’s current needs.
- Set payroll only after the actual renewal and completed elections. Use the selected medical, dental and vision plans’ actual premiums and approved employer contribution policy to update deduction amounts, contribution setup, plan/tier codes, and effective dates. Include dental and vision changes, coverage tiers and billable dependent changes in the payroll implementation checklist. Reconcile enrollment files and the first affected payroll; do not use the 3.4% base action or any posted renewal average as a deduction or budget assumption.
- Coordinate employee communications. Explain confirmed plan changes, pharmacy/provider checks, enrollment decisions, and when payroll deductions will change. Review Medicare-creditable status and any required notices for Medicare-eligible employees and dependents.
