2027 Medicare Part D creditable-coverage determinations for California employers
A renewal and compliance alert — not a rate announcement. For payroll and HR administrators confirming plan status, notices, and CMS filings before 2027 enrollment.

What changed. CMS's revised simplified determination methodology is mandatory for non-RDS group plans for plan years beginning in 2027. A plan must be designed to pay, on average, at least 73% of participants' prescription-drug expenses, replacing the older 60% threshold. For off-calendar 2026–2027 plan years, UnitedHealthcare says to use the 2026 methodology until the 2027–2028 renewal, then retest.
General administrative information only — not legal advice, a plan recommendation, or a rate or premium forecast.
Carrier status for 2027
Status is plan-specific. Always confirm the exact renewal plan against the carrier's current determination.
Official employer/broker pages list multiple 2027 Small Business plans that will be non-creditable upon renewal. Not all Blue Shield plans — check the exact renewal plan against the current list. Employers may keep the plan and issue the non-creditable notice, or select a creditable alternative.
UHC's official 2027 Small Business lookup says its 2027 California Small Business plans are creditable.
Kaiser's 2027 California Small Business carrier notice lists its plans as creditable.
Sutter's September 14, 2026 carrier letter says all 2027 small- and large-group plans are creditable.
All Commercial Small and Large Group pharmacy plans available for the 2027 plan year and in place as of September 15, 2026 are creditable. Plans built or modified after that date require confirmation.
A carrier-issued list identifies specified plan designs that "may not be Medicare Creditable." This does not apply to every Aetna plan — confirm the exact plan.
Notice and CMS disclosure deadlines
- Notify Medicare-eligible individuals whether the coverage is creditable or non-creditable.
- Complete the CMS online disclosure no later than 60 days after the start of the plan year, within 30 days after plan termination, or within 30 days after a change in creditable status.
- Annual individual notices are still due before October 15. A change in status requires an updated notice.
Which groups this affects
California statutory small group is 1–100 employees; large group starts at 101. Creditable-coverage status is plan-specific and separate from rating. It does not change the small-group single risk pool or the carrier-specific pooling or blending practices that may apply to some 101–300 large groups, and it does not mean an employer's claims alone drive its renewal.
Payroll and HR workflow
- Identify Medicare-eligible people: employees, dependents, COBRA participants and retirees.
- "Part D status verified" checkpoint: confirm the exact plan's 2027 status before any plan recommendation or employee communication.
- Retain evidence: save the carrier determination with the renewal file.
- Deliver the correct notice: creditable or non-creditable, matched to the final plan.
- Calendar the CMS filing: record the disclosure deadline for the plan year start, termination, or status change.
- Self-funded or level-funded plans: get the determination from the TPA/PBM, or obtain appropriate testing or actuarial support.
- Payroll and enrollment setup: change plan codes, deductions or contributions only after the employer selects the final plan.
